Fall Sports Physicals
SPORTS PHYSICALS
The PIAA requires one comprehensive physical per year for eligibility to participate in interscholastic athletics. The PIAA requires that the date on the PIAA Physicians Form Section 7 must be no earlier than May 1, 2025, for the upcoming school year. If the date on the form is incorrect the physical will be returned, and the athlete will be ineligible to participate until the physical is dated properly. We do not require any immunization information as is stated in the physical packet. We will not accept any physical paperwork that is not on the PIAA AND SHALER AREA forms that are downloadable at Titan website, under the tab more and then physicals information, or picked up in the athletic office at the High School. The website at the bottom of this page can also be used. .
The fee for all physicals taken at school is $10.00 and is due at the time of the physical. There will be a Functional Movement Assessment offered for fall sports physicals free of charge.
Checks are preferred and can be made out to “S.A.S.D.”
ALL PHYSICAL PACKETS MUST BE FILLED OUT COMPLETELY AND SIGNED BY A PARENT OR GUARDIAN TO RECEIVE A PHYSICAL. WE ARE ASKING THAT ONLY FALL SPORTS REPORT FOR PHYSICALS AT THE DESIGNATE TIMES. WE WILL BE PROVIDING PHUSCALS FOR WINTER AND SPRING SPORTS AT A LATER DATE.
FALL SPORTS PHYSICALS FOR ALL STUDENTS (GRADES 7 – 12) – WILL BE HELD ON: May 29, 2025, at the high school. All physicals will be dated after May 1t, 2025 Please report to the main lobby and you will be directed from there. Your report time is based on the grade you will enter in the school year 2025-2-26, so we do not have people waiting for long periods of time. You will not be permitted in the school before or after your scheduled time. Please allow for up to a 1 hour and 15 minutes to complete the process. See schedule below:

Seniors – 11:45 am
Juniors – 12:30 pm
Sophomores – 1:15 pm
Freshman – 2:30 pm
8th graders – 3:45 pm
7th Graders – 5:00 pm
